Use the Provider’s website to see what pre-referral information they need i.e self-referral forms.
Please then email your pre-referral information to secretaries.westernroadsurgerybillericay@nhs.net with details of why you would like referral. Tell the GP what symptoms you have and how they affect your life.
Children
