When a patient dies

Please make contact with reception so we can support you and help to make the arrangements needed. One of our GPs will complete the ‘Medical Certificate of Cause of Death’ (MCCD) or communicate with the coroners office if this is required. Once completed we will let you know as at that point you can book an appointment to register the death with the Registrars Office.

Asthma/COPD/Diabetes

Our Practice nurses provide ongoing reviews for patients with the above conditions. We will contact you to arrange this review around the month of your birthday.

NHS Health Checks

These are offered to patients over the age of 40 every 5 years. You will receive a letter when yours is due. The nurse will run through a review of risk factors for cardiovascular disease including diabetes and high cholesterol. Remember this appointment cannot cover things managed by GPs so you will be asked to book another appointment to discuss other things.

Smoking Cessation

We would love to help you stop smoking. Your chances of quitting are 10 times higher with the help of a Practice Nurse. Call to book!

Smears

You’ll receive a letter when your smear is due. If you have any concerns about having this done please speak to us.

Carers

If you are a carer and you need support, you could contact:

24 hour ECG and 24 hour BP

These will be booked as required by the GP. You will be given an instruction leaflet at time of booking which includes instructions on what to bring and driving.

Child Health Surveilance

8-week baby checks will be carried out by the GP. Please book for these when you register your baby.

Childhood Vaccinations will be carried out by the Practice Nurse. You will receive letters to remind you about these.

Autism and ADHD assessments for children

If you are worried that your child has Autism or ADHD, they require assessment by Community Paediatrics. In order to refer into their service, both you and the nursery/ school need to fill in assessment forms. Please could you have a look at the following website (and fill in the forms), then make a GP appointment so we can put your referral through.

Autism and ADHD assessments in adults

If you believe you may have autism or ADHD, our receptionist will book you in with our PCN Mental Health worker. They will do an initial assessment, and provide you with some questionnaires to fill in, before making the relevant referrals.

For support through the autism diagnostic process and beyond, you can contact SUMMIT. They can help with anything from extra support in the workplace to benefits or housing issues. They also offer Group and peer to peer support. Contact on info@summitservices.org.uk or 01255 429778.

Travel Advice/ Vaccinations

If you are going abroad and need advice or might need vaccinations, please fill in a Travel form for you and each member of your family travelling. Our nursing team will review your forms before booking appointments as needed for vaccinations. Some vaccinations cannot be provided on the NHS but they can give you details when they speak to you.

Shingles

This jab is recommended once between the ages of 70-79. Do get in contact if you haven’t had it and you are in this age group.

Pneumococcal

This jab is recommended once over the age of 65, and some people under this age may be recommended it by a hospital consultant if you have certain other conditions. Call to discuss if you think you need it.

Men ACWY

This is offered to young people going to University for the first time.

Flu

Flu vaccinations are received by the surgery in September and will be given in flu clinics running on Saturdays. More details available nearer the time.

COVID Vaccinations

Although the surgery was involved with booking the early COVID vaccines, these are now contracted to other agencies. Those who are due a vaccine can book online at https://www.nhs.uk/book-a-coronavirus-vaccination/do-you-have-an-nhs-number or call 119.

Vaccinations for people who are housebound are provided by ACE Healthcare Ltd. We cannot give dates/ times of when these will be given but if you are not sure if you/ your relative is coded as Housebound please call reception. 

Sick Notes/ FIT NOTES

You need to self-certify for the first 7 days of illness.

After this time our doctors can write you a certificate if they believe it is necessary- please book an appointment.

When this note ends you are ‘Fit for Work’. There is not another form to declare this.

Wart Clinic/ Cryotherapy

Dr Patel runs regular cryotherapy clinics to deal with veruccas, warts and other minor skin conditions. Please call for a review with a doctor if you feel you may need to be booked in for this service.

Minor Surgeries

Dr Patel also performs minor surgeries for certain skin conditions. These can be booked after an initial appointment with a doctor.

Non-NHS Services

Please call reception to enquire about non- NHS services for example fit to fly letters, to whom it may concern letters, insurance company letters. SUCH A REPORT WILL TAKE UP TO 14 DAYS TO COMPLETE. There are charges for this work which you will find below. Please bring cash or cheque as we are unable to take card payments at reception.

Hospital Appointments

Please note that we are unable to contact hospitals to chase/ change appointments, or to ask for sooner appointments. We understand that hospital phone lines are often not answered quickly, but we do not have another way through. Please persist with calling them directly.

If you feel your medical condition has changed, do book an appointment with a GP for review.
